Payment Policy
OUR DENTAL OFFICE IS A FEE FOR SERVICE PRACTICE, WHICH MEANS DR. ALVAREZ DOES NOT PARTICIPATE WITH ANY INSURANCE COMPANY, IS NOT A PROVIDER WITH ANY INSURANCE COMPANY, AND THE OFFICE DOES NOT ACCEPT DIRECT PAYMENT FROM ANY INSURANCE COMPANY.
PAYMENT IN FULL IS REQUIRED AT TIME OF TREATMENT.
Dental Insurance
If you have a PPO type dental insurance that allows you to go out of network, you may still be able to use your insurance and receive reimbursement from the insurance company for your dental treatment. Our office will be happy to electronically file all insurance forms for you to your dental insurance company. Dental benefits are an agreement between the dental insurance company and the source of your dental insurance (your employer usually). Our office has no control over any of the terms of your dental insurance including, but not limited to your yearly maximum, treatments covered, waiting periods, deductibles, exclusions, or any other determination made by your dental insurance company. Our office is merely submitting claim forms as a courtesy, and it is still your responsibility to pay for your dental treatment at time of service.
